Understanding Prognostic Markers In CLL Care
- The term “prognostic markers” refers to lab results and genetic tests meant to help predict how chronic lymphocytic leukemia (CLL) is likely to behave.

“There are clinical prognostic markers, and then there are biological prognostic markers,” Dr. Kieron Dunleavy, a medical oncologist and hematologist at Georgetown Lombardi Comprehensive Cancer Center in Washington, D.C., tells SurvivorNet.

Dr. Dunleavy emphasizes that how a patient is actually doing — their symptoms and everyday lab trends — often carries more weight than molecular test results, especially for patients who are considered stable.“If patients are doing very, very well, if they have CLL and their lymphocyte count is not changing that much, and their platelets are good, and they’re feeling well, and they have no symptoms, even if they have poor molecular prognostic markers, the clinical markers are often more important,” he says.
These clinical measures may include:
- Lymphocyte count trends: Rapidly rising lymphocyte counts can signal more aggressive disease.
- Lymphocyte doubling time: This refers to the time it takes for the lymphocyte count to double. A doubling time of less than six months is associated with more aggressive disease and may prompt consideration of starting treatment.
- Platelet count: Doctors will monitor whether this drops below 100,000 per microliter and how quickly that decline occurs.
- Symptoms: Enlarged lymph nodes may indicate evolving disease.
What To Know About Molecular Markers
Once clinical factors are considered, doctors also examine the molecular and genetic features of CLL cells. Several of the most commonly tested markers include:
- 17p deletion or TP53 mutation: This has traditionally been considered one of the clearest markers of a poorer prognosis. However, Dr. Dunleavy notes that with modern treatments like BTK inhibitors, “it’s not as robust a prognostic marker at all… BTK inhibitors can overcome the negative prognostic impact of it.”
- 13q deletion: This is generally associated with a more favorable outcome.
- 11q deletion: The evidence here is mixed. “Some studies show that it’s good, some studies show that it’s bad,” according to Dr. Dunleavy.
- Immunoglobulin mutation status: CLL with a mutated immunoglobulin gene is generally associated with a better prognosis than CLL with an unmutated (non-mutated) status.

Modern Therapies Changing What Markers Mean
One of the most important points Dr. Dunleavy makes is that a marker’s prognostic significance isn’t fixed, as it can shift as treatments improve. Many newer, targeted therapies are able to overcome the negative impact that a marker like a 17p deletion once carried in the era of chemotherapy.
That means a “poor” marker on a lab report doesn’t carry the same weight it once did.
“I have had patients who will really get fixated on that, and clinically they’re doing great, and there’s no evidence of progression, but they have a TP53 mutation or a 17p deletion. They really get focused on that,” Dr. Dunleavy says.
He encourages patients to keep the full clinical picture in view.
“We have to look at everything. The clinical course and the clinical prognostic markers are also hugely important,” he adds.
In other words, prognostic markers are one piece of a larger puzzle — useful for guiding treatment decisions and monitoring — but not something that should overshadow how a patient is actually feeling and doing day to day.
